 From you files:
%!PS-Adobe-3.0 EPSF-3.0
%%Creator: The Open Group PostScript Print Server

Do you have all the needed patches? Have you tried the Xprint server
instead of the Sun server?

The xp_ps_spooldir_tmp_Xprintjobs prints to a file any way, in
/tmp/Xprintjobs. You should select you proper printer and then print to
file. See if that makes a difference.

Both printed fine for me on an HP 8150. [2] is pinted using Courier for
the main text and Times-Roman for the header and footer. [5] was printed
using Times-Roman for everything.

I noticed in the edited versions, [3] and [6], you left the font
selection the same. This uses the builtin fonts, which may not match on
the Cannon (though they should be in the normal PS default set).

How is the print spooler setup? I'm guessing that the problem, and not
Xprint, since the Xprint output prints fine for me. I know resolution
setting have been problematic at times, but you files are set for
300x300 which should be OK. You might try changing the default to
600x600 or the printer default to check.

In /usr/X11R6/lib/X11/xserver/C/print/attributes/document (or the
equivilent in your setup)

*default-printer-resolution: 600
